Officer Leroy Azlin, along with two other officers, was dispatched to a scene where a female was threatening suicide with a knife. As one of the officers attempted to distract the subject, Azlin and a third officer worked their way into a better position to stop the woman. At the time Azlin thought it was best to make his move, the subject turned the knife at the officers. During the struggle to get the weapon free from the woman, Azlin received two thrusts from the knife, striking him twice in the chest. The blade from the hunting knife tore through the uniform shirt but was stopped in the Second Chance® vest Azlin was wearing.
